Required Forms

Each camper will need to fill out a Health Form and a River Bend Liability Form prior to attending River Bend camps. Campers that are signed up for any Adventure is Out There camp will require additional waivers for field trips. They will be provided to you via email.

If campers are signed up for multiple camp sessions throughout the summer, only one set of forms is required. Forms will be kept on file for the duration of the summer. Forms will be emailed out at the time of registration. If you did not receive your forms, you can access them below. If you are attending and Adventure is Out There! Camp, there may be additional forms. Watch your email for more information.
